Dr. Phil Davidson and his staff want you to be an active participant in your care, striving toward optimal outcomes and making knowledgeable health care decisions. Dr. Phil Davidson sees new patients for all varieties of problems related to the knee and shoulder. He provides second opinion evaluations and often sees patients that have unresolved orthopaedic problems. Many patients seek treatment with Dr. Phil Davidson for their knee or shoulder problem when they have been told that there is little hope, or no treatment options to resume an active lifestyle.

Dr. Phil Davidson is presently accepting new patients and currently accepts most major insurances.

 

All scheduling of appointments is done by the front office staff. All surgery scheduling is done by Jennifer Meyer. She can be reached at 435-615-8822 or at jennifer@heidendavidsonortho.com.

It is our policy not to see patients without scheduled appointments. The office schedule is filled well in advance; in some cases, people have waited months to make an appointment. We understand that it is tempting to bring someone with you in hopes that they can be seen as well during your appointment time, but in fairness to those who have waited, we are not able to see anyone without a scheduled appointment. We appreciate your cooperation in this matter.

• Your insurance card or other health coverage information
• Walking shorts for knee examinations
• X-rays and/or MRIs not older than 12 months. Do not rely on the MRI facility or doctor's office to send the films. Pick them up yourself.
• Clinic notes or operative reports from your referring physician that relate to your condition.

Payment in full is expected at the time of your visit. Exceptions include workers' compensation, Medicare, and preferred provider contracts (co-pay only). Check your insurance information to identify whether Dr. Phil Davidson is a preferred provider for your policy. For your convenience, we accept Visa, MasterCard, cash or check. Please contact the billing office if you have any questions or concerns.

Should you have any questions prior to your visit, please feel free to contact our office at(435) 615-8822  and we will be happy to assist you.

If in the future you need to request medical records from our office, please be aware that it could take ten days to two weeks before you receive the information. We will make every effort to fulfill all requests for medical records as quickly as possible.

Dr. Phil Davidson operates out of Park City and the Salt Lake City, Utah area.

Many patients come to treat with Dr. Phil Davidson from outside of Utah and from other countries. We have organized logistics to support coming to Park City or Salt Lake City for surgery. The arranged services include: transportation, lodging, outpatient and inpatient surgical services, "home" or hotel based nursing, "home" or hotel based physical therapy, and coordinated care with health care providers near home. We also maintain a roster of recreational opportunities for accompanying family members. Park City and the Salt Lake area are beautiful to visit year-round, with affordable options to choose from.
